Chelsea are set to re-sign former midfielder Tiago on a free transfer.
The 33 year old midfielder is now a free agent, and Mourinho is ready to reunite with one of his ealiest signings.
Tiago signed for Chelsea in the summer of 2004 from Portuguese side SL Benfica, just weeks after Mourinho had took the job for his first spell at Stamford Bridge.
However he failed to make an impression at the Bridge, as Frank Lampard and Juan Sebastian Veron were picked as the centre midfield two.
So much so, Tiago, then aged 24, signed for French side Lyon that summer, and has since played for Italian side Juventus, before signing for Atletico Madrid in 2010.
Tiago made 24 appearances for Atletico last summer, as he helped them to a first La Liga title in 18 years.
