Rafael Benitez is keen to be reunited with the Liverpool defender and hopes to do a short term deal in January.
If the Denmark international was to move to Napoli then he would link up again with Spanish goalkeeper Pepe Reina.
Reina is currently on a season-long loan from Liverpool and the Italian side, who will face Swansea in the last 32 of the Europa League, are looking for defensive reinforcements.
Agger has played just nine times for Liverpool this season after suffering a freak gym accident and was rushed back too soon. And Martin Skrtel, Mamadou Sakho and Kolo Toure have been played ahead of the Dane.
Speaking last month Agger said he was keen to stay at Anfield and fight for his place.
He said: ‘Competition is what this club has been about and should always be and that is a good thing because we want to win something.
‘I’m not the type of player who needs an explanation, I just carry on with my thing and try to do everything to get back into the team.
‘It’s been a bit longer than I hoped for but when you get the chance you need to do everything to keep your place.’
